totally can trigger unmarried, I got with ginger, and then with gust in the living room, immediately got the two timer achievement after she saw me smooching on him lol

I actually think you can only get it unmarried, as NPCs will refuse the Knot (and therefore you can't have 2 boy/girlfriends) when you are married

I have Ginger as a lover and accepted Dr Xu's request to go steady so that I can trigger the achievement but nothing is happening. However many dates and cuddles I have with Dr Xu and however much I kiss Ginger neither of them confronts me. I have even done it in the presence of the other but still waiting.

I may have to hand the heart knot to another person and hopefully there will be a higher probability of a confrontation.

In order to get the Two-Timer achievement, while unmarried and with multiple lovers, you need to ask one of them on a date. The other lover has to catch you while you are dating. So let's say you took Ginger out a night stroll and you wanted to do the Astronomer date! You decide to make your way up running to the direction to the Abandoned Ruins 1 hills, passing the bench Dr. Xu will normally sits. He will then catch you dating with Ginger and the achievement should be unlocked. Make sure you're kind of close for him to catch you.
