What is the purpose of "custom" option in graphic settings?

"Custom" just means that you edited one or more of the graphics settings beyond the preset graphic setting. (Like, if you set the graphics to "high" but edit a setting, it changes to "custom"). It's actually a feature in most games that allow you to customize graphics settings.

The lack of customizable graphic settings is really bad in this game. No options for bloom, antialiasing, etc.

Indeed. But the strange thing is that the custom option is not in general when you set a parameter for all the graphics settings like other games, but is a specific setting of graphic detail, after ultra you can choose custom but no extra panel or options appear?!? i use ultra. Some reporting that choosing custom lower the fps in some area and speed up in some other... and the framerate option will reset at 144 even if i choose 30 or 60 as soon as i left the graphics panel...
Maybe future patches.
