it's a joke, but still maybe it will help you somehow.
At least that's how it worked for me.
You were right, You have to put the rocket precisely in the front opening of the launcher. It works 100%!
I had problems with this in the closed beta last month. in the Discord I suggested for the devs to have the ammo counter to show if it's loaded or not since the counter just shows total ammo.
I also suggested to simply just have the RPG model sticking a bit out of the front of the launcher barrel to show that it's loaded and ready to fire, but I never got a response back, so either they got busy and forgot to reply back or didn't see my suggestion. :/
All they really need is have the brigadier general character show how to load it, cause once you figure out how to use it. It is super easy. Reach over your shoulder stick a new rocket in the front pull trigger and BOOM.
