The Patch is free because Mangagamer will provide for every release on steam a free patch
Mangagamer is not like Sekai Project, where you pay for patch for a altered version that is only 18+

I'm being dumb no doubt; But if we buy it through steam how would we go about asking MG for the patch? I have an account with them from Ozmafia but I prefer buying through Steam where available.

The page has all the trials and adult patches, on the upper left of the page you can filter the files by category, game or developer.
At the moment I see that there's no patch for bocchimusume, only the trial of the game, I guess the patch will be available to download upon release.
