Three out of four times I finished this game, she just gets pregnant and retires somewhere.
Two times I let her make her own choice - she decided to leave. Quest Failed.
One time I just killed everybody because I was Evil. Quest failed.
Does failing it mean her scar disappearing but being filled with anger and vengence? Because she still passes the trial there.
But maybe someone else has more information.
You can tell her to leave your lands, but there is no resolution to her questline that results in her death at the House at the Edge of Time.
As to how to get the Hellknight ending for her, you have to essentially affirm her beliefs that beauty is worthless. Don't make her apologize to the poet in her second quest, chase after Fredero in the first, and in the third, massacre everyone at the Temple of Prisms (the third is the big one). If you never get the "good" resolution to her quest, where the scar is removed and she gets +2 to WIS and CHA, that's the ending she ends up with.
