You are a druggie, and it will take more then a week of sobriety for some of that damage to begin to fade.
If you want to know what/how the portrait may change, here are some spoilers:
Firstly, you can try to study your appearance in the mirror of your room to discover the source of "The Expression". With a sufficiently good roll you can remove it, although this just makes you look like a bit of a sad sack...
Second, on day 3, you can navigate over the water lock and get a free room from the old lady at the docks. Inside there is a razor and a mirror. You can opt to remove those mutton chops and go clean shaven. Again... this doesn't so much make you look better, as it does expose some more of the damage. But hey, variety is the spice of life they say!
On my first playthrough I went the whole game with the starting amnesia 'unknown' non-portrait of blurriness just because.
Ahh understood, well guess it's a little too late for me, I'm in day 2 now.
