Also, the ping you see in the server browser is not your ping to the host, but to steams servers. To view the ping within the server you need to look after you have joined. In some cases a 25ms ping in browser is a 300ms ping in game.
Do they have above minimum specs to play the game? Or they might have some packet loss going on? It sounds like an issue I've run into with previous builds of Barotrauma. But those could be "solved" with a strong host machine. Not that I'm saying this is the case. Only spit balling.
This can be true also. Slow host machine makes desync, as does slow client. They would have to be under min specs for that though I think. Or on Mexican internet.
First recommendation:
Disable texture compression from the graphics settings of the game.
Second recommendation:
1 - Install and enable Lua for Barotrauma
2 - Install and enable Performance Fix
3 - Host a server with LuaForBarotrauma ServerExecutable
4 - While you are in the lobby press F3
5 - Type "install_cl_lua".
6 - Restart the game
7 - To run a server with the mods we just installed, enable the Lua for Barotrauma executable when hosting, without this they will not have the performance improvement.
This applies for Server and Client. So if you have both installed the optimization will be complete.

To run a dedicated server with LuaForBarotrauma you must go to the installation folder of the game and run LuaDedicatedServer.bat
You must configure the server from the serversettings.xml file, but not from the game installation folder, you must do it from LuaForBarotrauma, which is installed with mod Workshop.
