how does item quality work?

Unsure how it works,at first I suspected it was both skill of the item in question and also talents,but I am unsure

item quality are effected by skills low skill or same skill requirements makes normal quality items. 10+ more than the required skill makes good quality items. 40+ more than required skill makes excellent quality. no more than that.if you want to up it more you need talents to get it into masterwork quality item.
