To answer your question correctly, my personally scariest thing (after playing this game for 1000+ hours) was when for whichever reason I abandoned the submarine/shuttle, and had to swim to relative either safety (if you have a chance), either just accept that you will die, i.e. be eaten alive, out of oxygen etc etc. This feeling of loneliness, not knowing where you are going, complete disorientation, the only thing you see is the light of the helmet shining into nowhere and the only thing you hear is the sound of your character breathing, not even water, no monsters, all of your crew is dead for any reason. Regrading something like this, the game gives you just unimaginable emotions, and to be honest, some of these little things, which I can tell are scattered around the whole gameplay, is what kept it always interesting to me.
Regarding my own question, considering the annoying part, is that once in ruins (or if they break into the sub/shuttle), the mobs, monsters, or creatures have a pretty bad mechanic regarding attacks/movements, this sort of stuff, so their hitbox can literally run away faster than you can aim at them (oh and yeah if you dont have any weapon like stun baton, you can just accept that you will die). The creature's attacks are immerse and mercyless, they bite and stun you twenty times more than you will be able to travel/run/swim in meters. Worst part is that you have basically nothing to do about it. I know most of the subs (all default and some modded) by hard, so I know which and where their objects, secret stashes etc. are, to kind of protect myself, say give me another minute or two to play.
Other than that, hope this slightly answer your question.
i could count that as a scary thing i could accept, so your saying that the loneliness and paranoia is the scariest part of the game?

A more recent example for me was getting paralyzed on a Thalamus for the first time. Before, I was quick enough with a plasma cutter to dispatch the Leucocytes. I had heard that they caused paralysis, but I didn't have much experience with paralysis anyway. I got attacked, and I started to slow. I tried to make it back, but I stopped moving on the edge of the wreck. And then...
I lay there, trapped in my own body. I didn't know if rescue was coming, how much Oxygen was left in my diving suit, or if anybody saw my last radio message requesting help. Utterly alone and helpless.
the "fight" took near an half hour trying to rescue the ship, but with 5 crewmen and only a few oxygen bottles left... losing one by one the entire crew - that was scaring.
