Dunno really what can honestly be scary. If I were you, I would have asked another question - "What is the most annoying one"; but well, thats different

To answer your question correctly, my personally scariest thing (after playing this game for 1000+ hours) was when for whichever reason I abandoned the submarine/shuttle, and had to swim to relative either safety (if you have a chance), either just accept that you will die, i.e. be eaten alive, out of oxygen etc etc. This feeling of loneliness, not knowing where you are going, complete disorientation, the only thing you see is the light of the helmet shining into nowhere and the only thing you hear is the sound of your character breathing, not even water, no monsters, all of your crew is dead for any reason. Regrading something like this, the game gives you just unimaginable emotions, and to be honest, some of these little things, which I can tell are scattered around the whole gameplay, is what kept it always interesting to me.

Regarding my own question, considering the annoying part, is that once in ruins (or if they break into the sub/shuttle), the mobs, monsters, or creatures have a pretty bad mechanic regarding attacks/movements, this sort of stuff, so their hitbox can literally run away faster than you can aim at them (oh and yeah if you dont have any weapon like stun baton, you can just accept that you will die). The creature's attacks are immerse and mercyless, they bite and stun you twenty times more than you will be able to travel/run/swim in meters. Worst part is that you have basically nothing to do about it. I know most of the subs (all default and some modded) by hard, so I know which and where their objects, secret stashes etc. are, to kind of protect myself, say give me another minute or two to play.

Other than that, hope this slightly answer your question.

there is nothing scary about the game when there's like 12 people randomely running around and yelling memes while trying to stab eachother with scalpals, there's no end game and there's really nothing to do, there's often no teamwork to get anywhere and the only goal of the game is death.

i could count that as a scary thing i could accept, so your saying that the loneliness and paranoia is the scariest part of the game?

Swimming around, in the dark, with only the diving light to guide you. It's really eerie just diving in the dark with no light source and being completely alone. There's also a terror not knowing if there's a fractal guardian or trap in the next room you're trying to play. But for conventional spooks, nothing is scary. Except for endworms. Those things are twice the size of the Berilia.

My friend wanted me to get this game so he let me play it when I was over at his place, I was doing a mission and I noticed that a room had water in it on the status monitor. I walk over and open the windowless door. IMMEDIATELY a mudraptor jumps on me and I actually yelled out loud and scared my friend. It was one of my first experiences with the game, I bought it after I got home.

The first time getting swarmed by Leucocytes, not knowing what they are. The first time we got hit by a Thalamus. Plunging into the abyss chased by Hammerheads as you frantically try to fix the ship. Hearing noises in the next room of the alien ruins... Lots of creepy moments in this game. Be sure to play with friends and up the difficulty as you get competent. Even on Hellish the game is very beatable in a good sub with an experienced crew, but things will still happen to challenge you from time to time.

I've had a few moments where the respawn shuttle crashes and I have to franically swim out, only having a general direction of where the sub is. Swimming like that, alone, not knowing where you're going, and getting no response on the radio can really make you feel hopeless and afraid.

A more recent example for me was getting paralyzed on a Thalamus for the first time. Before, I was quick enough with a plasma cutter to dispatch the Leucocytes. I had heard that they caused paralysis, but I didn't have much experience with paralysis anyway. I got attacked, and I started to slow. I tried to make it back, but I stopped moving on the edge of the wreck. And then...

I lay there, trapped in my own body. I didn't know if rescue was coming, how much Oxygen was left in my diving suit, or if anybody saw my last radio message requesting help. Utterly alone and helpless.

Oh, or that time I was swimming around, with absolutely no idea of where I was.. and then a shuttle came speeding at light-speed towards me! It was flooded, and a Charybdis had gotten to it! I desperately turned on my scooter, but it was too late.. The shuttle struck me, and I was injured severely. The shuttle then hit the sea floor, crushing me into a wall as it moved diagonally. The Charybdis had a light snack that day.

my scariest "thing" was not a creature, just simply the fright of high explosives detonatet in front of my diving suits emptying all the bottles instantly and also instantly flooded 80% of my sub.

the "fight" took near an half hour trying to rescue the ship, but with 5 crewmen and only a few oxygen bottles left... losing one by one the entire crew - that was scaring.

The scariest thing is without any doubt the clowns in this game.. When they "honk" and look at you... Jeez.. That's something to think about i mean.. If a clown not is scary in the real life i donno what else..

the real terror of the sea

Clowns aren't scary. A Captain that's aligned with the clowns and refuses to let you shoot them on sight, that's the real terror.

One of the startling things to accept about Barotrauma is when the legacy creatures are going to be fully implemented back into the game, and what else the Europan waters will churn out.

I was really glad to see "The truth is out there" event especially.
