You can also bury them in a grave, but red skulls naturally reduce the quality of a grave, while they do not count negative for zombie efficiency.
If you have BSS, then making perfect corpses is very easy, and just upgrading them in general is much easier.
If you don't have BSS, then part of getting better zombies is strictly luck based. Once you have the cultist perk that lets you see the skull counts on organs, it does become easier, but it's still all luck based on what quality of the brain, heart, and intestines will be (all other parts have a set value and never change).
I also believe that, without BSS, you must remove blood and fat from a corpse in order to make it closer to perfect.

No. Those can never be removed. With the surgeon perk, you will never do those mistakes again.
The way you remove a zombie with that is to throw them it the river. You will be able to recover from that mistake later in the game anyway so it isn't a big deal.
It is short for the DLC Better Save Souls. Red Skulls are negative traits in a corpse. Gold and Silver injections can turn 3 red skulls into 3 white skulls. Since Lye gives you +1 red and white skull. Before you start injecting a body it should not have any Surgical Mistakes and never have more than 2 red skulls. The best you can make a corpse is 26 white skulls, which gives a zombie 65% effectivness.
This is because BSS allow you to modify organs to add red and white skulls to organs (not skulls and not bones). It is expensive, but it is well worth it in the late game.
The best way to know if you how many skulls an organ have is to get the Cultist Perk for 300 blue points once the Inquistitor have asked you to collect dark organs for him. With the Cultist Perk. The skulls are no longer hidden from you.

No, it means you need better organs. Heart, brain, and intestines need to be swapped out for better ones. Ideally you want each to have 3 white skulls (red skull count for zombies doesn't matter outside of injections).

With or without DLC you should collect the organs with 2 or better 3 white skulls and exchange the zombie organs that have fewer than 2 white skulls. This brings significantly more efficiency.

No. I do not yet. I could create zombie with 9 skulls currently, but the 2% zombie! oh well.
