Anyway to retire a zombie?
I made a couple with 2% efficiency back then, and I was hoping I could just keep improving them...but no. Can't do multiple injection seems?

My only option is to throw them in the river? What! No one warned me about that before!

You can improve zombies like other bodies by injections and regular autopsy. Just keep in mind that you need some red skulls on them before you can apply silver/gold injections. Gold requires 2 red, silver requires 1 red skull.

You can also bury them in a grave, but red skulls naturally reduce the quality of a grave, while they do not count negative for zombie efficiency.

You can also swap organs on zombies so if its efficiency is low better organs can make it a better worker.

Just to note, I think it's Lye that ADDS a red skull, so you shouldn't need more than 2 red skulls on a body prior to injections. For zombies it isn't important how many red skulls they have, but for corpses it of course does.

If you have BSS, then making perfect corpses is very easy, and just upgrading them in general is much easier.
If you don't have BSS, then part of getting better zombies is strictly luck based. Once you have the cultist perk that lets you see the skull counts on organs, it does become easier, but it's still all luck based on what quality of the brain, heart, and intestines will be (all other parts have a set value and never change).
I also believe that, without BSS, you must remove blood and fat from a corpse in order to make it closer to perfect.

does a red skull allow multiple injection?

can you swap surgical mistakes?

what is BSS? How far away I am from having red skulls anyway. I haven't seen one yet.

No. Only one of the same type is allowed.

No. Those can never be removed. With the surgeon perk, you will never do those mistakes again.
The way you remove a zombie with that is to throw them it the river. You will be able to recover from that mistake later in the game anyway so it isn't a big deal.

It is short for the DLC Better Save Souls. Red Skulls are negative traits in a corpse. Gold and Silver injections can turn 3 red skulls into 3 white skulls. Since Lye gives you +1 red and white skull. Before you start injecting a body it should not have any Surgical Mistakes and never have more than 2 red skulls. The best you can make a corpse is 26 white skulls, which gives a zombie 65% effectivness.
This is because BSS allow you to modify organs to add red and white skulls to organs (not skulls and not bones). It is expensive, but it is well worth it in the late game.

The best way to know if you how many skulls an organ have is to get the Cultist Perk for 300 blue points once the Inquistitor have asked you to collect dark organs for him. With the Cultist Perk. The skulls are no longer hidden from you.

How do you recover if you can only inject it once? So in the end, the only reasonable option is to throw low efficiency zombie into the rivers?

No, it means you need better organs. Heart, brain, and intestines need to be swapped out for better ones. Ideally you want each to have 3 white skulls (red skull count for zombies doesn't matter outside of injections).

Here's a link to the wiki page explaining how to make a 12 white skull corpse, which is the max value for base game.

You can only inject one of a kind of the injections. Glue, lye, silver, gold. Silver and gold cannot be injected if the zombie does not have at least 2 red skulls (for gold) or 1 red skull (for silver injection), because silver and gold have to remove 1 or 2 red skulls. If they are not in the body, the injection cannot be applied. You can remove the skull (the head) to have one more red for silver or gold injections. But I guess it is not your main problem, because zombies most likely have enough red skulls.

With or without DLC you should collect the organs with 2 or better 3 white skulls and exchange the zombie organs that have fewer than 2 white skulls. This brings significantly more efficiency.

Do you have the cultist perk to see red and white skulls of the organs already?

No. I do not yet. I could create zombie with 9 skulls currently, but the 2% zombie! oh well.
