You actually have to go to a alchemy station and start mixing. A failed attempt will give a goo output that will give hints to what failed and how to do it better.
The health potion you need is crafted from 1x Life Powder and 1x Health solution. or 1x Health powder and 1x chaos solution.
Life powerder can be gained from processing Intestine or Sulfur. You need to have research them first.
Health Solution can be gained from processing Carrots, Hiccup grass or Cabbages. Research first.
Health powder can be gained from Pumpkin, cabbage or crop waste
Chaos solution can be gained from Moths or Bat wings.
You can learn recipes the following way:
Mixing them yourself.
Buying a scroll from Clotho
Doing the Stranger Sins DLC that give a alchemy scroll as a reward for most items you find.
Scrolls will randomly unlock a recipe for you. However that reward might not be as useful anymore with Save a Soul DLC since they added a lot of paint.
What I recommend and what I do is to just look at the wiki. There is way to many possibilities to mix for you to just randomly mix the things correctly.
