Possible fix for controller disconnecting mid-game
This has worked for me for the last 2 days, I have had 0 controller disconnects while ingame:

Open steam settings, go to controller

click the general controller settings

Make sure your controller type is checked for the config (for me it was xbox config)

Make sure "shutdown controller after idle" timer is set to NEVER, for reasons unknown it was set to default of 15 min shutdown for me

It seems like that's a big picture thing, and MHW wasn't always starting in big picture mode for me which made it seem random. After stopping steam from disconnecting my controller on "idle" whatever that means (apparently that meant actively using the controller for 15 min straight?) I have not had a single controller disconnect issue since.

@choksquaker are you using Steam Input for Yakuza 0 or Sekiro? Neither of those apps are opting players into Steam Input like MH:W does. If you're experiencing those issues without using Steam Input here could be some kind of HW issue going on like having a fraying USB cable, etc.
Necro'ing the thread because now I'm having a similar issue except it's not based off any amount of time rather when certain conditions are met, IE, certain combos connecting in Monster Hunter will disconnect the controller momentarily, or doing a certain action in Sonic Advanced and Split/Second. I've tried changing it to other controller setups but it doesn't seem to be working.

Tried all the different controller profiles on Split/Second all lead to an eventual disconnect and an often disconnect at that. Trying to identify the controller on the controller menu disconnects it as well. Currently testing it out with a new controller, that might have been the issue all along.

Are you using a switch pro controller? I seem to recall there was some issue with those disconnecting due to vibration.

Nah just an Xbox controller. I thought it was a steam client issue because the disconnects were so specific but it might have just been the controller..
Holy thread revival batman. Ive been using steam for a few months. NEVER had an issue. Now as soon as the guy says "go" in Dirt2 my xbox controller loses connection. The weirdest thing is that while im sitting at the startline revving its fine?! Same in the menus. I cant get my head around that. Similar issue in project cars 2. Constant droppoffs all of a sudden. The games are unplayable. This has to be a bug??
I'm having the same issue. As soon as I launch elden ring, the controller disconnects.
Mine ended up being caused by a crap usb extension cable. It was the rumble causing too much draw and making it lose connection
Setting the shutdown timer to "NEVER" solved the issue for me.
I'm using DualSense (PS5 controller) on Linux. Try it in Metal Gear Rising and Batman Arkham Origins.
Not really related to MHW but I came here from google search. Maybe it will help someone else :happymeat:

Shutdown controller after idle is an option in big picture mode. You'll have to be in big picture mode and then go into the steam menu and settings from there.

Yeah I also have the same problem! It never occurred before for me until now. I was playing Jedi Fallen Order and in mid-fight, my controller disconnected and Cal (the main character) started going in circles and sometimes even forward or backwards! I had to pause the whole game from keyboard and keep on unplugging and plugging the controller back until it could work.
I also have this issue, i don't know if anyone noticed, but the controller for me at least starts working again if i tab out of the game and get back into it. Maybe this might help figuring out the problem. Other thing is this problem seems to happen only for bluetooth with me and only on steam. The controller i am using is a 8bitdo pro 2.
Also having this happen and think it might be related to Pro controllers needing higher voltage than the USB cable is providing. If you shut down at night, try unplugging it from the PC and charging it or just leaving it to the side. May be draining or not charging while plugged in to the PC. I know the PS5 dual sense edge def has different voltage requirements from the standard, but not sure on the others. This might not correct it, it's just a theory.

Other theory is that it's associated with Unity games that require server connections to send data, like analytics. Look in your local low folder and change all the default analytics to false, and reverse I think one of the true settings to false. So if that server it's sending has a bad ping, you can cut that out of the equation as well.
Digging in this thread like a Mechanicus Adept on a Necron tomb world. I’ve got my Steam Deck hooked up to a TV using a dock. My Steam Controller is connected using an official dongle. It will disconnect every 5-30 seconds with the Steam logo on the controller flashing.

If I leave the Steam Controller dormant for 10 minutes then brush the touch pad, it will register mouse input for a split second before refusing any further input on Dusk. Sometimes it works for minutes at a time, sometimes I have to shut off the controller completely and re-connect it to prompt input.
