Guitar Controller? Now I think about it, are those even around any more? I remember the Guitar Hero fad.... Feels like an age ago.
What were they called again? Oh, right. HOTAS, was it?
I think the only thing I could feasibly manage would be playing in VR.
I played Dragon's Dogma in VR for a while. It was a surreal experience. VR and 3rd person games are just... surreal. Like you're just someone kinda tall following behind the character.
Let's drive one monster into each other instead of a wall bang :D

Replying to the OP:
Both are viable, but controller should feels more natural without much need of key rebinds since it's the way MH has been designs with. M&KB usually need some key rebinds to feels natural.
Also: Joystick on the controller allows 360 degree of direction adjustment, so it's better for aiming a melee weapon with.
Meanwhile M&KB let you be more precise with Bowguns & Bow aiming, unless you turn on a gyroscope on your controller.
...But Steam's gyroscope is super weird and not as well built as Capcom's gyroscope on Rise for Nintendo Switch so let's just stick to M&KB for aiming on PC.
This over 30 y/o "kid" can play several hours with KBM just fine without pain. Actually, I can play way longer with KBM because controller tends to stress MY thumb more.
It practically saved me from getting carpal tunnel syndrome, and my wrists aren't even fragile.
PS: In fact, I did get carpal tunnel syndrome once, due to playing this game way too much. Never had issues after I started using the mod mentioned, however. Turns out the key reason was straining myself too much because I was pressing the same bloody button 20 times for the game to register ONE whole input. The mod lets you just hold inputs, and they get accepted as soon as the game starts accepting them. This massively increases how responsive the game is, and lets you do some things like sheathing and consuming an item with just holding one button. Or shooting full auto with bowguns.
I use a controller in many third person non shooters, including this game, however what your talking about has nothing to do with controls, and everything to do with bad posture, mouse and keyboard does not make you lean forward, its a natural reaction to wanting to be closer to the screen, both KB+M and controller users suffer from forward posture and back problems due to this.
Maybe you have a better chair, but most of the time keyboard and mouse force the arms forward, thus "forcing" a bad posture or straining the back a bit.
Can be because of many reasons. Maybe you don't play a lot, don't write a lot. Can be your bones are very sturdy and you have a healthy back + a healthy lifestyle. Every person has some weak point: some people don't have to wear glasses but have weaker backs or the opposite etc.
Revisiting the game now... I actually decided to go with mouse and kb because my controller has seen better days, and I decided to try great sword and honestly, I don't think I'll go back to controller. I much prefer m+kb.
So... I think both are good options tbh. Give each a go in the training area and see what works best for you!
Edit - should also add that I find the default binding scheme for m+kb to work really well tbh
I actually do have some issues with mild back pains. But here's the thing, whenever I'm on the couch I often tend to be in a worse position but office chair sort of "naturally forces" me to sit more straight, like something that happens without me thinking it much. I also realize that it is also a me-problem for putting myself to a bad position on couch, and is something I could (and should) fix.
Well, I talk about my viewpoint as the absolute truth, because people who are sensitive to things know better. Someone who does not experience a bit of back pain at all will not notice that they are actually hurting themselves until it's too late. I already had extensive use of keyboard and mouse in my life and talk from experience, it does not do the fingers and wrists any favor, especially as one gets older. In most cases it also forces the arms to go forward wich is a mild strain on the back. Thus controller is the better option for long sessions of gaming in the long run, especially if one uses keyboard & mouse for other things than gaming.
It's different naturally for someone who may not use keyboard and mouse most of the day and just use it for gaming 1 hour daily, or for people who have bones as strong as a Barroths head, but that's rather the exception than the rule.
