To do this, press ESC in-game, press the system (gears) tab, Options, the Controls tab (second one from the left), Keyboard Settings.
Then you'll see the keybind menu open up and you can rebind the game's controls as you want. I recommend going to the Melee Weapons tab and making sure the Main Key for Guard/Special Action/Fire Claw is Ctrl, Display Weapon or Slinger Aim (Press Once) is C, and Display Weapon or Slinger Aim (Hold) is set to V.
Aside from that, to explain the hammer charge system in its entirety:
You hold Ctrl to go into charge. During charge, Stamina gets slowly used up and you advance between 3 charge levels, starting at 1 and going up whenever your character flashes. You can move freely while charging, but doing a roll or cancelling the charge will reset your levels.
By releasing the charge key, you let your attack go based on what charge level you were on. Charge 1 just releases a basic attack that can combo into Hammer's usual options. 2 makes you dash forward and lets you combo into Hammer's usual options much faster. 3 depends on if you're moving or not. If you're moving, you do a cool spinning attack, if you're standing still you deal a beefy hammer slam.
If you're sliding down a slope (or move into a "jump-off" surface like the mushroom walls of the ancient forest), all these attack options are overridden for the amazing Midair Spinning Bludgeon, one of the best, most fun moves in the game. It's environmentally dependent, but a really, really good attack.
There's also Power Charge which you do by pressing rightclick during a charge. This makes your hammer start glowing, while its glowing it has an attack buff. You should try to maintain this buff as much as possible, as it's essentially free extra damage. It doesn't expire over time, but will instead vanish when you sheathe your weapon or are knocked back by a monster.
