I'm not implying that some runners may or may not use external apps for stagger tresholds etc., but on PC you never know.
RNG elements are often involved in the speedrun times anyway, and if speedrunners can deal with stuff like Super Mario Bros 3's insane random elements in Any% Warpless playthroughs of that game I think they can deal with the HP variations in Monster Hunter.

ATs and Behemoths are great because no HP rolls , but optionals are a lot faster than those so you could pump out more attempts at those if you wanted to run optionals.
Speedrunning, even on a "casual" level, is tedious RNG regardless of fixed HP or not, because the monster AI is random and you spend more time resetting and preparing for the quest than you do actually fighting most of the time.
