Report a translation problem
On your first run, don't kill any NPCs and make sure you buy everything they sell. Even after buying everything they have, don't kill at the very least Big Hat Logan, as he will drop a couple of things you will need if you complete his quest and kill him in his arena fight afterwards.
Try to do all your weapon farming on NG to avoid the annoyance of stronger enemies (particularly important for the Channeller's Trident), and either make or find a decent checklist cause there's a lot of things that are easy to overlook.
As for covenants, there are only 2 which require very specific conditions. One is the covenant of Sunlight for which you have to either help in coop or get your faith relatively high just to unlock it. You'll need it unlocked anyways, because you want to eventually use Gwyn's soul to get a miracle. And the other one is Darkwraiths for which you simply need to kill the 4 kings before placing the lord vessel.
As for why you need to play the game twice and get like half way through the third playthrough, is because Gwyn's soul is needed for 2 things and Sif's soul is needed for 3 things. So you basically end your all-achievements run right at the giant blacksmith getting Gwyn’s souls turned into his sword and Sif's into whatever you're missing.
Lastly, if you're not decent at parrying and Gwyn can whoop your ass with ease, just make sure Solaire makes it alive during your first 2 playthroughs. His weapon and shield doesn't count for achievements so there's no particular reason to take them unless you want them. So if you do want them and aren't confident about beating Gwyn, just kill the poor dude once you find him during NG+2. And, if you don't want to do NG+s you'll have to request someone to trade you the additional boss weapons, you'd still need to get the Sunlight Spear miracle, so at the very least you'll need to get to the statue for the Sunlight covenant on NG+.
