Should ı spam purifying flames after ulti ?

In most cases yes. It heals more than it damage. And your ult double your heal. The only problem is with casting it right before it wears off.

Also you may use combination of skill 2 and 3 to heal and not damage because otherwise it will be very inefficient.

Medusa is an exception. Never spam purifying flames on friendly Medusa. Go nuts on enemy dusa though
You can spam your flames on teamates under glimmer, 2 skill or BKB.
