You either call gg to capitulate or to commend your opponent for a game you really enjoyed.
Oh yea, it means "good game"
When I see anyone writing "GG" in the beginning of the match or in the middle (like 5 to 20 minutes) it means that the guy is either giving up or is trolling and right after he/she says something like "noob team".
Well, at least that's how I see it.

Oh they have, but mine had spit on them. lol
It is considered unmannered for the winning team to call GG prior to the official conclusion of the match if the losing team does not call it first. This is called an "offensive GG" and is frowned upon because until the match has officially concluded the winning team has no authority to claim their lead is insurmountable.
If the match officially concludes, either side is free (and encouraged) to say GG in any order.
