To provide a simple example, imagine if I wrote code that calculated 2+2 every second and then every 10 seconds it calculated 256x256...it would stutter every 10 seconds if something wasn't powerful enough to hold up.
I generally recommend limiting your fps to improve fluidity. You can also monitor your CPU and GPU power/temperature and utilization to see what it hitting "max". Could be that your CPU or GPU is temp locked and one of them is hitting max. Generally CPU/GPU temperature max temp is 85-100C, but it depends on the specific chip so that might not be super helpful to you unless you do more research.
Also, I recommend limiting your fps to 2 below the max frame rate of your monitor ALWAYS...even if you can run at max frame rate. It will drastically lower the latency of the monitor. It has to do with buffering in the monitors and you could research that more...or just trust me.

I doubt I have calculation problems, I have a 4080 and an i7 13700k 32gb of RAM.. the thing persists even if I block the fps under 240.. that's exactly how I saw that the 165fps are not as fluid as on the 165hz monitor..
I trust you, that's not the problem, it's mostly Dota that does this trick and maybe some other game... even with the 165hz monitor if it went below 165 it did the same trick... and I don't understand why

I probably have the solution for you. But before that, were you using the settings of windowed or bordered Dota 2 instead of extensive full-screen? DirectX or Vulkan? Do you mind to share all your video settings out by using screenshot?

Unclear information. Direct X11 and all set max? Means you did tick all? What about other adjustments such as your fps and rendering percentage? Hard to understand your situation as I think a screenshot can fully explain all.
I guess your max fps and dashboard fps settings have issue to hold the fps that could be supported by your good GPU.

No doubt the Dota 2 game system can be ruined by so many players if you greed for abuse so much. Blocked!

Try the following steps:
1. Check to synchronize your desktop fps with the fps of your game application, Dota 2
Reason/why: When you are switching between your desktop and game app (especially minimizing or using overlays such as NVIDIA or Steam overlay), probably affects the smoothness of the GPU to keep the balancing of fps up or down. So, make sure the fps are close to each other for GPU to have the least working behind.
2. In-game video Settings such as the "Use advanced settings" can be played without these 3 which don't have much impact on the business but may cause smoothness issues of the screen with GPU, the World Lighting, the Ground Parallax, and VSync. When I play Dota 2, I disable them only.
3. Game Screen Render Quality can be set at 100% but it's not fully recommended to adjust it at 100% for all time if your PC isn't smooth with the strongest hardware (higher GPU doesn't mean stronger). Concerning no.1, you need to have smooth fps with your desktop. Thus, you need balance and stable fps with each other, the recommended fps can be 60/120/144/165 (no need to be high till 240 which consumes too much power). I usually use 165 Hz only for both the Max. fps and Dashboard fps. So far no issue as my desktop is set to 165 too. This means that my application switching is smooth and has no lag.
4. Regarding the "Display Mode" you selected, it seems you are using the 'Borderless Window' with the maxed power of your monitor resolution. This doesn't seem balanced for your monitor to maintain its quality to generate good graphics for you because the "Windowed" and "Borderless Window" modes are for players who prefer the application of Dota 2 to run at a "smaller" size on a high-resolution monitor. Your monitor has a good high-resolution of 2K and if you want to play at balanced and stable fps, it's highly recommended you select the "Exclusive Fullscreen" mode for your game application too.
Reason/why: When your game app runs at "fullscreen" it means it runs into full borders where all the resolution/pixels of the game app will be pushed into the maximum use to synchronize properly with your monitor. Still, please note that this setting will consume more power than lower resolution such as 1920*1080 Full HD resolution to be used on the 2560*1440 2K monitor. If you want to use 1080 Full HD, then switch the mode to either "Windowed" or "Borderless Window", and your game size will be adjusted to smaller-look (more balanced). FYI, I am using a 2560*1440 2K monitor also, but for smoother run and fast loading, I always run the app at 1080 Full HD only with "Windowed" mode at 165 Hz fps. If you have high resolution monitor, it doesn't mean you must push your game app to follow the same resolution unless you prefer for bigger-look.
5. I am not fully clear about the settings of "NVIDIA Reflex Low Latency" because you have to ask some real professional here, or you may try inquiry to Steam Support team who usually will explain to you what it means by sharing your machine information too. Sometimes I do send inquiries and have good returns of answers. To share what I know and understand about this, the best I know about it is, probably this is an adjustment to control your GPU to work "friendly" with your desktop as I explained earlier in no.1, regarding the desktop fps and the app fps. I usually use it with "enabled" (no boost) because I think when it is enabled, then the GPU usually will work or optimize with your desktop fps/resolution for any action such as minimizing, screenshooting, or recording. You must understand, when you start an application, your monitor is running with a "new big layer" to be handled for you. That's why the GPU is very busy and consumes much energy if your adjustments are not "close" to the desktop, yet you keep making actions especially minimizing to use of the Web Browser, using Friend Chats, etc (best is to use the Steam overlays to reduce all these actions).
6. If you feel all the steps above are not working, then I guess your PC Windows Security might have an issue behind already as Windows Security has "Ransomware protection" service which is very strict to prevent certain applications from accessing the PC files. That's how certain applications may face crashing or graphic glitches because the protected folders are being monitored and defended by Windows Security by preventing any access from the unknown application including the Steam application unless you adjust to allow Steam to access through the protected folders.
So far, all are what I experienced for balanced and stable gaming for Dota 2 because my Dota 2 gaming never crashed before while I was gaming except when I was using Vulkan settings that might have graphic glitches (rare). Hope it helps.
