It really depends on what hero you're playing. Usually I'd go one main item and then mkb, or if you normally buy shadow blade on the hero (like on slark for example) get silver edge asap.
edit: I just tested it, and Treads + Armlet + MKB gives more DPS than Treads + Armlet + Deso.
edit2: correction: deso seems to be less valuable than mkb against targets with 10+ armour.
if it were me I'd build mkb basher ac against pa.
-mkb - you won't miss as much
-basher - bashes will proc more reliably since you have mkb around to reduce evasion. also stops pa from attacking you temporarily when it triggers.
-ac - more attackspeed means more lifestealing, more bashing, and more mkb magic damage procs. and gives you armor to survive pa physical damage on top of that.
lifestealer wins against pa earlier, pa has the advantage by mid game, but if you reach late game with mkb, abyssal, ac, (bloodthorn for really long games) then lifestealer should have the advantage again.
edit: fixed typo
I bet one of the reasons Lifestealer had less than 40% after rework winrate precisely because people thought you needed his shard, another being that he no longer is a first phase hero. Now he's sitting at 47% winrate next other carries.
