A long time ago, I played WoW. They had a great keybinding scheme. I managed to implement quick cast and self cast without removing normal target casting all on one bar. I could do this because I could bind spells to shift 1, ctrl 1, alt 1, and plain 1. I could even do ctrl-shift-1. I had every item I carried and every spell I had bound to a button or a combination of buttons with all three of these casting types enabled (e.g. heal was on 3, I could self cast with alt-3 and quickcast with shift-3). And it was wonderful! I want to do something similar in Dota.
Unfortunately, since Dota 2 has a RTS style interface, shift and ctrl are both unusable for this purpose, leaving me with only one modifier for my hotkeys (alt). Dota also decided to not let us choose a self-cast button and set it to the same thing as their only modifier (unless they gave us the option to change it without rebinding alt, that would also be an answer to this query. I don't see that in the settings). Currently, I have to choose between the three things I am constantly using. Which one do I want to be slow? Precision casting, quick combat casting, or self casting.
