Their stats drop gradually when you're at the settlement.
If you spend a lot of time at the settlement crafting, identifying, stockpiling, activating neurons etc. make sure you eat, drink and sleep before you leave.
...(and make sure nobody has an item in their right hand that will keep them from eating, drinking and sleeping with you).
To grab leaves and swing hold Space when passing thru them. Don't try to time the grab, just hold Space on the way down.
If you spend a lot of time at or near the settlement make sure the clan is fed, watered and rested before you leave or you could respawn in a new primate who's in terrible condition.
