On hard mode, enemies have more health, do more damage, and you can't lock on to enemies.
On very hard mode, all enemies can kill you in one hit.
If you do die, you will have to do that bit again which is about 30-60 minutes. - very first time i played the game, it took me 3 attempts to do.
If you want to ease in gently though, you can set as easy, and then when you get to the point you can save, set difficulty back to normal. There is no autosave in the game, but, once you can save, you can save very frequently, and at plenty of terminals in the game world.
