I like this idea a lot. Urinary tract infections do exist after all, and I have never allowed my character to do such a thing in more than 1,350 hours of game play. It is a carryover from real life, I guess... ...and I sometimes find myself yelling at the screen when certain streamers seem to ignore the "I am going to let it go" warnings that pop up. If you are reading this, you know who you are. Gross.
This would motivate players to take better care of their characters in game. It is kind of like not repairing your footwear, clothing, and storage items even though you have a sewing kit on hand, but that is another issue.

Actually, although it is generally more of a female concern given anatomy, both men and women can get potentially nasty infections (including drug-resistant ones) from contact of their "nether regions" with poop, including their own, especially if the poop is in contact for a while.
Remember our friend E coli? It lives with us well in the gut but is a real pain in the butt when it gets into the urinary tract (pun intended).
This is why you always teach girls when potty training to "wipe front to back" and to "prioritise good hygiene". Thanks, mom.
In addition to pain/burning during urination and a low fever, more severe UTIs can present blood in the urine and high fever. Left untreated, UTI's can spread into the bladder, kidneys, and in men... ...well I think you know where. Other organs can be affected in the worst cases and there have been deaths associated with acute infections.
And then there are yeast infections. I will leave that one alone!

Yes it can happen in rare occurrences in rl but do we really need to take Scum that far!
We are talking about a lengthy prolonged exposure or other extreme medical conditions to contribute to it!
On average 12% of men experience a UTI at some point in their life.
Causes: STD's, prostate issues, diabetes, kidney stones, catheter's and Dehydration.

The wolves can now be spawned on you more often due to the smell of the blood.
The shark also from blood in the water when swimming! Although this one is highly unlikely in real life. Not quite enough blood for the attraction effect.

I love this description , sorry just had to say it. With everything already in the game wouldn't it be a bit much adding more diseases? Then there would be the issue that no one would play a female character if they get more diseases. I for one am not comfortable with playing male characters. I would shelve the game to collect dust if this were the case.
Not wiping properly after going to the bathroom. This can move E. coli from your poop to your urinary tract, causing a UTI.
