Negativity aside, I have heard some tactics to avoid/reduce this. Wear headphones, you can hear direction and footsteps sound different then random puppets. Approach POI's and towns like your gonna die. Store the good stuff at base and fast in and outs without looting the whole place. Wear a bullet proof vest, apparently crucial this update. Never run in POI's/towns. Walk/crouch to observe and hear better. Assume a NPC is going to spawn in directly behind or in front of you at any moment. Trying to enjoy a beer while playing? Best not. By the time you get back from the bathroom a puppet has spawned in, sniffed you out in your 3rd floor room by yourself and killed you. Good thing I put negativity aside.
