This game is the hardest game I've ever played and I've played a lot of games in my time - it's not for everyone.
If the writing, lore, and story doesn't interest you, I'd say this game is not for you. All you're left with is some very punishing survival mechanics that will test even the most hardcore survival-game fans.
If you get to day 4 and after, you'll start receiving payment for your service. Better reputation also nets better rewards, so it might be a good idea to quench your bloodlust on criminals rather than on innocent people. Holding "C" gets you into sneak mode, which is blatantly OP. Sneak behind a criminal with a knife, then with two sneak power attacks he is down, and you have potentially a buncha loot in your hands. Just try to make sure there aren't a dozen more criminals around the corner to get revenge for their friend.
I haven't actually beaten this game. I think the farthest I ever got was day 4, but I suck at it so much I don't know if I'll ever be able to get farther, even with everything I've learned, but thanks Lynceus for telling me about sneaking around, I'll have to try that.
Playing again and managing to do much better than before thanks to your newfound knowledge is arguably one of the most enjoyable parts of the game, though, so as long as you don't mind going through it again, it's not a bad option either.
Anyways, I hope you are able to get through the game, and have fun while doing so. It possibly has the best written story I have ever seen in a game, and I think you'll definitely find it worth it. Good luck!
The thing is that I find the game fascinating, but I just don't feel like I'm very good at it. I'm tempted to mess with the settings and just lower the build up of my hunger, but I also want to play the game as intended. I really want to see more of the game and figure out what's going on, and to see it all for myself instead of just watching someone else play.
If you are struggling a lot with hunger, feel free to lower it a little bit. I think it is better not to, but if it gets you through the game, it is worth it.
If you don't mind me asking, though, how is it that you go about the first few days? Do you buy the revolver, save your money, or spend it? What do you spend it on? and so on
Like I said, I tend to trade in a way so I can build up my reputation with people, and after playing through the beginning several times, I now know how to finish the day with some extra food, but it tends to fall apart by the end of the third day. I also always make sure to buy the special bull since knowing this game it will help me out eventually. From what I can tell, saving money isn't really worth it, and instead it's better to save enough for the bull and then spend the rest on stuff that'll help with trading for food.
I also play the game with the kids and the boxes, and make sure to follow the rules since again, I figure sticking close to the superstitious beliefs of the culture are likely to help in this game. I know where they are and as I go about the town doing stuff I make sure to visit them.
Another thing I'm not too sure about has to do with the theater. I seem to remember it being required in the first game, but this time it doesn't seem to be. I make sure to go each time, even though I've seen these things quite a few times after trying again and again, since I'm not sure if there are consequences for going or not.
Now that I know what I'm doing, I also make sure to do that thing with the candles in that house with Notkin and Khan, but it doesn't seem to really do anything as far as I can tell. I just do it in the hopes that there are some actual effects from doing it, and I get to loot some stuff from the house as well.
Although getting extra reputation is always nice, and becomes really useful later on, I'd say between getting some more of it in the beggining and getting some more value for your trading, it is better to do the latter. You'll get many opportunities to increase your reputation, and in the beggining of the 1st day I'm pretty sure it is pointless since it inevitably goes into the gutter at the middle of the day.
From what I know, going to the theather, other than giving you a better look at the story and hints at what is to come (and having an achievement tied to watching all the pantomimes), is only really useful because it reveals the location of the Dead Item Shop immediately after you leave it. The Dead Item Shop is really important, and can single-handedly save you and other characters by selling food, valuables, and Schmowder.
(P.S.: buy all candy wrappers, when the time comes)
Other than basically setting up your matchstick supply for nearly the rest of the game, it does have a great effect on the game: if you complete the quest, you prevent the Crude Sprawl from being infected/burned at the beggining of the Plague. It is one of about three quests (I believe) that are able to prevent an entire district from being infected, and I'd say it's more than worth it both roleplay/gameplay wise.
I mostly get the tourniquets for trading rather than healing, since I've found men who want them. I also try to upgrade my equipment, so now know to save some of the stuff I find the in garbage rather than just trading it all, with the expansion to my carrying capacity being the most important thing to me, since that just means I can carry even more stuff.
I was also surprised to find that the last time I played this game was in November. I'll probably just start the whole game over again the next time I try this seriously, but for now I've been replaying Planescape Torment.

My last playthrough I managed to hoard enough bullets that I became the freaking punisher. XD
When this is not the case, however, I'd probably argue that the revolver is really not worth it. You can avoid pretty much all the really tough combat scenarios, and the only one you can't avoid does not let you have weapons with you anyways. The revolver is probably good if you mess up and end up with a bunch of robbers coming at you when you have little stamina, but then again, it takes so long to pull it out that it might not be a life-saver even then. You can also get a free shotgun later on if you are able to kill three enemies (not even with as much health as the common enemy) at once, so I just feel like buying the revolver isn't that worth it. Considering how hard it is to get ammunition anyways, it might be better to just focus on buying food and get shotgun ammo for later on, and use a knife and sneaking as your way of dispatching foes. You will have to learn how to make good use of melee combat anyways, for the aforementioned section where you have to fight enemies without having weapons.
In the first days, just try to learn as much as possible about where resources come from and who they are useful to - trash cans, NPCs, houses, children's caches, dead bodies. The sooner you understand what items are needed for what (parts for item repairs, crafting, trading with children/teenagers/men/women/shop owners/dead item shop, etc.), the easier time you will have because there is always a way out of a sticky situation, you just need to know where to look. There isn't a single item in the game that isn't useful somehow. You basically have to learn the town's culture and it's a central part of the game. To get as many advantages as possible later on, make sure to *always* finish hospital work and maintain good reputation around town.
