Blood Symphony Achievement Won't Unlock

I beat the game in Overkill Mode. Why can't I earned it?

Although the achievement is not specific in the conditions of what is required, I suspect the game must be completed in the normal mode rather than the + mode for those achievements to unlock since the Overkill mode does make things a lot easier.
