Like said above, throwing crouched helps a lot at not getting detected. Also throwing from a great distance into the building where the plague heart is is a solid strat.
Another way is .. i think Programming. Lets you use drone strikes. Long as you don't toss the smoke under the roof of the house then the explosion from outside nearly kills a Plague Heart on Lethal. IT ALSO ATTRACTS ZOMBIES, so quickly take out the heart in some form or however you like (of course).
Just plop them down, run back a ways, and set them off to explode. EZPZ.
*Not sure about Lethal though.
I found that it took too many to kill it before the zeds were storming the place from hearing all the explosions. Plus it takes so many pipe bombs to kill it, that too many slots were taken up in my pack. For me, the faster the better.
The way you do this is to park the car with the front touching something that prevents zombies to latch on to the hood, for instance the house.
Then run inside while dodging any zombies. Place the C4 while looking out for not being grabbed from behind. Dodge more zombies and run outside of the blast distance, but with clear line of sight of the Heart. Shoot 4 rounds into the Heart, blow the charge, throw the Thermite Grenade at it, and dodge-run back to your car. The Thermite will eventually kill the Heart. Get in and start driving. Smash the Feral with the rear of the car.
Now go back in and pick up the loot. This whole process should take you less than a minute in total.
Another method that lets you kill the Heart before the spawned zombies get to you is the BML-40 grenade launcher. This is even faster and easier (still doing the dodge-run thing), but it's more expensive.

Beyond what weapons to use, some strategies to try are as follows:
-It's usually a good idea to bring snacks/energy drinks/stimulants to a Plague Heart since you will be doing a lot of running in between attacks.
-Unless you're playing on Lethal-Combat difficulty, don't bother killing the plague zombies surrounding the heart unless you're stealth killing them before beginning the encounter. They should all drop dead following the destruction of a Plague Heart.
-Plague Hearts will often be clumped together on difficulty levels where the Plague Heart count goes above 20 hearts or so. If you destroy one, the zombies summoned to protect it will die as mentioned previously. As a side effect this also gives you some breathing room to start attacking the other Plague Mart in the strip-mall or across the street without having to worry about roving Plague Zombies. Combine that with the high-tier drops from a Plague Heart and you can get the ball rolling to kill four, five, or six Plague Hearts at a time.
-On the other hand, attacking a Plague Heart will almost always spawn two hordes and usually a freak to deal with. The two hordes are inconsequential since you should be running from them instead of wasting consumables fighting them, but that freak can take you off guard if you're not paying attention. Ferals aren't too much to worry about since you should get very used to killing them in a single head shot by just playing the game naturally (be able to identify the scream, start running to make distance, force the Feral to run in a straight-line towards you, line up the headshot, easy money), but Bloaters and Juggernauts are a pain in the ass if the game decides to spawn them where you can't easily find them.
-Sometimes Mysterious Broadcast missions will spawn where a random character not tied to a settlement will be gung-ho to kill a Plague Heart. These guys are almost always just decoys for you to lay down mines and blow the abomination to kingdom come, and don't get attached. On the plus side, more often than not they're Red Talon or something so you can pick their SCAR or whatever off their body when they die from over-confidence.
-On the Meagher Valley map, the first Plague Heart is almost always going to be in the Swine & Bovine just down the hill from your first settlement. If you get lucky and find some guns early, you can climb the ladder to the roof of the restaurant and shoot out a skylight for a stupidly easy first Plague Heart and consequently first high-tier drop.
