I can't really explain your first point, because sometimes if you're near your base regular zombies and freak zombies can still enter your base and maim and/or kill your survivors.
For your second point, I found that aiming (holding right click on your mouse) and walking towards the ladder automatically has your character magnetize to the ladder, so you don't walk past it, fall off, and break your player's back near a group of zombies. I hope that helps.
They can not. They only exist in a 100-200m radius around you. A more likely explanation is that one of his NPC survivors got Blood Plague from a Siege or something, but the game doesn't always warn you of this. You have to manually check for it.
The ladder bug idk about it but i know about the health link bug but there is no way if you know what your doing that you gonna die so easy ....
Playing on the hardest difficulty uhm kinda but is not a game that is rush and run and as always you can have a team mate with you friend or AI

Unlike the first game there's no one to sometimes help you, even if you have a follower.
When up on cell towers, especially, I just shuffle (move slightly) towards the ladder and they grab it when they're supposed to.
I don't know why ladders are such a problem in some games. I even have the same issue in Watch Dogs Legion. Shuffling towards it, rather than walking or running seems to help.
I do get the odd issue with other ladders, though frequently this is due to me hugging the edge of a roof to try and scope out and kill the freak below (usually a feral or 5) and often times i'll end up jumping off the roof right next to the ladder. again, being right on the edge, and off center on the ladder is usually the issue.
Any time I WANT to go down a ladder, it works.
As for the random death. Who knows about that. Since the latest update I've not really had any sieges. Try and take out the plague hearts ASAP and if they wake up, they are my primary focus.
I usually play on standard because I was never bothered about the harder difficulties, but I initially bought the game from the windows store when it released. I got the steam version from a humble bundle where there were other games I wanted, so a fortuitous thing.
Since starting it up on steam (and learning that basically everything other than like survivor pool etc) has been transferred over. My achievements, my unlocks, my previous legacies etc, I've started my first ever Nightmare run and working on doing my legacies for that. First map was Drucker, just about looted the entire map and have 1 plague heart left to take out. No survivor deaths. Going good.
I love this game, but I can't wait for SOD3 - though it seems theres been no news on that for a LOOOOOOOOOOOONG time now.
The solution is simple. Have a simple button promt to use the ladder. That way you dont jump off accidentally. You know when to press the button. that is how many games with ladders do it safely. Never fell of in those games.
Also the "co-op" is terrible.
But yes, if SOD2 had a [Climb Down] prompt, too there'd likely be less deaths in communities :-)
Hell, juggernaut spawns in general seem to be a bit higher than necessary overall, seems they're just everywhere nowadays. I wouldn't complain except for the fact that they seem to take so much effort to kill for virtually no reward, especially since they seem to respawn so quickly.
