If you can manually reload, I've run into that with various characters. I think what's happening may be something like this -- You fire the last round in the mag, but the next shot is the one that won't use ammo, so the launcher doesn't auto-reload (because it doesn't need to). But then when you fire the next shot, the auto-reload doesn't trigger because the mag was already empty. It's like the launcher skipped the point when it would normally reload, and the auto-reload isn't triggered again after that.
FWIW, it seems like the problem is triggered by manual reloads, and as long as I always let the launcher auto-reload, I don't run into it. When the problem does crop up, I have to keep an eye on the ammo and be prepared to manually reload if the auto-reload doesn't kick in.
If you actually can't manually reload, and the only way to get the launcher to reload is to swap weapons, I don't think I've seen that. That might be some combination of the glitch that I described above and "Smaller, Lighter, Faster".
