the story is the same.
It's definitely worth it if you really enjoy the gameplay and character building/progression. Otherwise probably not.
If you want additional content, the "main" DLCs are good for that if you havent played them yet.
The story missions will be the same, but your enemies will be tougher. When you complete TVHM, you will unlock the third playthrough, Ultimate Vault Hunter Mode, UVHM. Again (much) tougher enemies, same missions.
Extra story. No. There are the various DLC's which are best started in TVHM though.
Normal mode, your loot is capped at 30. TVHM, capped at 50. UVHM capped at 72, if you have upgrade pack #2.
All the extra vaults you see? Hopefully in future Borderlands... :)
Is it all worth it? Depends on you. Completing 1st playthrough you have learned to walk. TVHM is schooldays, UVHM is the Real World. That is where the fun is. Your characters get to be Powerful and Dangerous in UVHM. :)
Although higher difficulties have some nice quirks, like the multipliers for using corrosive weapons on robots are higher, and enemies have greater resistance to incorrect weapon types. When it isn't a measly 2/10ish ~ you have to think on your feet a little more. And the last 20 skill points (tvhm) tend to personalize your charachter way more than the first 25, and stop making the most important thing just your death match and aiming skills.
