That is the only DLC not included in the Handsome Collection, as it was developed and released long after the HC was released. It will need to be bought, as it has become an essential part of the game if you want to reach [max level] and do the optional OP levels.
The earlier version of the game was capped at 72, but when Lilith's was released, the game cap was raised to 80. Lilith's has the additional 8 levels, so is a kind of Vault Hunter Upgrade Pack #3. Get it on sale if you can as 2K got greedy with this one, and the new price is +50% higher than any of the other 4 DLC's.
Also save Lilith's for last, and do all of the other DLC's as well as Tales first. Lilith's has spoilers to Everything.
Well, I'm playing BL2 at the moment, and I have it. I never bought this. In fact the HC is the ONLY bunch of DLC that I ever bought, as I bought the original game when it came out, along with the pre-order content, including the Mechromancer, which came with The Premium Club. Is it possible that they added it to my DLC because I had purchased the pre-order package, and I got it automatically because I had? All I know is, I never ordered it.
The Handsome Collection will only add the stuff you don't have already and the price will be adjusted at checkout. The Premium Club will not have been involved I suspect.
A way to check if you have Lilith's is to see if you are offered the chance to create a new level 30 character on the main character select screen. Lilith gives you this choice now.

I haven't experienced this, but I think that the only way around this is to join someone in their game, before they reach the part of the story, and go from there. So same playthrough rules apply here.
There are other soft locks that can happen in the DLC - insufficient testing being the cause most likely.
