You need a save file from Borderlands one. It needs to be in the right place. BL2 is going to follow the normal save path that BL1 uses in order to check.

If you don't have that, or can't get it in the right place for BL2 to see it, then your alternative is to use a profile editor (and edit your Profile.bin file) to unlock it.

You play Borderlands (the first one) and it will create a save file when you save and quit.

Borderlands 1 save-files should be here:
Documents\My Games\Borderlands\SaveData

You don't find it, if you played the game, it will find the saves. If you have not played the game on the current system/OS install, then run the game, your saves should be brought in from the cloud. If you have the cloud off, start a new one and play long enough to get to Zed's place and you should have a save file by then.

edit: You don't NEED to find your saves for Borderlands 2 to see them.
However the above post has the path.

Maybe go to your library, select Borderlands 2 and click support. The only other thing I can think of is a cheat and is not selective, unlocks all of the cosmetics or none, profile editor.

Most head/skins can be unlocked using a Profile Editor.

Bordertool2 can do that. Gibbed cannot (as far as I understand).

It's still more fun to fight for them in my opinion, but to each their own.

You can't really fight for this one lol, it unlocks if you have a bl1 save and he does, yet it's not unlocking. What else can he do?

Open a ticket.
Do without it.
Use the profile editor to enable ALL cosmetics, it's not selective.
