Why is this rated 18?

I played this back in 2012 and I don't remember anything that would call for an 18 rating other than shooting dudes in the face.

Curious as I would like to play through the game with my child.

Its ESRB rating is "M17" which in BL2's case means the content's generally suitable for ages 17 and up because it contains intense violence, blood and gore, sexual content, strong language and use of alcohol. It's not the far more restrictive "AO" rating which is pretty rare in games. These ratings aren't federal laws anywhere ~ ESRB means them for parental guidance if you need or like guidance. I've played M17 games like the BL series with my kids and grand kids since they were in their early teens.

Violence, sexual themes, strong language. As to kids playing it, many do. As an adult you decide, some don't want the kids to hear or see such things, others are fine with it.

There are sexual themes. alcohol use. Violence. And alot of dark themes of murder if you pay attention to the story or back story. But for a kid that just wants to run around and shoot stuff, level up, gain new gear. It's just a fun game.
I really don't think kids take this stuff seriously or are heavily influenced by it honestly.
There are alot more suggestive themes on public television than can be found in B2 so ultimately I wouldn't worry.

It all just "cartoon" anyway. That's what I like about B2. It is fun, but doesnt tkae itself too seriously.

Look at cod kids and then look at people who play other games.

I played jaws when I was a kid growing up and loved all of the gore and blood there was.

There's some graphic parts, especially the Bloodwing battle, and certain cutscenes. Some of the dialgue can have heavy sexual content in it, too (I'm looking at you, Moxxi, Jack, and Scooter).
