For the motion tree, you need to get down to Converge for crowd control.
The first link is what you would need for co-op support. Get that then add some more damage in the other trees.

You could opt to put those 5 points in Immolate to Helios if you'd like. And if you were to get the Nurse class mod, that would further the healing power provided by this build.
Putting on the Siren or Cat class mods (again, if you get those) basically increases the DPS of this build...the Cat ever so mildly more. But I usually just rock the classic legendary Siren mod. Tends to be a better overall mod to use.
If you play on normal/TVHM you will do fine with any build. Res is too good to not to use it in coop but that's your only "must have".

I'm not a Melee attacker, full disclosure, so this build is centered around increased gun damage. But if you are, let me know, and I can help you with a build that does maximize close combat.
Weapons:
Maya is obviously an elementalist, and my favorite gun is Moxxi's Good touch, which is easily obtainable by giving her tips. The healing factor isn't as high as other guns of hers, but this build allows for many sources of damage at one time to maximize your healing! The variant you want to use has the round Maliwan handle, which has the highest damage, highest ignite chance, and shortest reload at 2.5 seconds. Any variant with a reload higher than 3 seconds will only compromise your damage output, even if you've invested in several skills that increase reload speed.
The Grog Nozzle is also an essential gun for UVHM. The high slag chance and high healing potential makes for less time slagging the enemy and higher survivability! In a tight situation, all you need to do is peek around the corner and fire at an enemy, and Cloud Kill will be the damage source that heals you. It's instantaneous!
Other than these two guns, you're free to choose whatever guns you want! Anything with an element is helpful!
Now to my choices in Motion, Harmony, and Cataclysm:
The damage potential of the skills I've chosen in Cataclysm, based on the many hours of experience I've gathered from playing as Maya, are the highest. Half of the enemies in BL2 are armored, so the fire nova produced by Backdraft and the incendiary damage from Immolate in Fight for you Life mode are only disabilities in those situations. In Fight for your Life, all you need is one bullet to touch a stationary enemy and Cloud Kill will save your ass big time! (Less time your allies have to spend saving you!) The only reason I've invested in Helios is because it does damage each time you phaselock, and to reach Ruin. Blight Phoenix requires you to be within close proximity, requires you to make constant kills to remain activated, and from my experience doesn't have high enough damage output.
This is where things become flexible! Thanks to meticulous research into the Harmony tree, I've deduced that there is no correct number of skill points to invest in the skills Restoration, Elated, Sustenance, and Life Tap. Everything is based on what your team needs and your playstyle. Res is an obvious choice for saving your teammates. Scorn is optional, whether extra slagging is your thing or not. Sustenance makes it so your teammates can focus on attacking rather than saving your life. Life Tap is only essential if you choose not to use Moxxi guns. Restoration is for situations where you enter Fight for your Life out in the open and you need to keep your friend alive; depending on how many people you're playing with, you'll need less or more points in this skill. (With only one teammate, I'd invest 3-5 points. With 2 or more, I'd invest 1-2.) Elated is only necessary to invest a lot in if you like to phaselock high health enemies to drag out Phaselock's additional bonus effects from Wreck and Elated (which is an ideal choice in my experience). Any extra points can be invested in Mind's Eye for a little extra damage!
The skills I've invested in Motion contribute to crowd control, extending and spamming phaselock, and overall gun damage. Fleet is a skill you only need to invest a few points in, Inertia and Ward are not what I would consider essential for COOP/DPS, but when alone the extra survivability is helpful!

When playing solo, you have many points to invest in other things, like Ward, Inertia, Fleet, Mind's Eye, or anything you like since you only have yourself to to take care of. Feel free to have fun with it!
Gear for your Maya:
Ideal shields are the adaptive and spike shields!
For the COOP/DPS build, a Nurse class mod of any quality, or a Legendary Binder class mod will work, for both contribute to the teams survivability and your damage!
For the solo DPS build, purple quality Witch class mods, or the Legendary Binder class mod work.
Proficiency relics are the best choice to continue spamming Phaselock. Bone of the Ancients also has excellent Action Skill cooldown rate and additional elemental damage, and are obtainable from Legendary Loot Midgets.
Grenade mods that work are anything elemental. I choose to use the Contraband Sky Rocket for its greatly increased damage, but its exclusive to those who have specific extra content.
