It becomes extremely difficult, depending on your gear and the degree of patience and smarts you use on each run.

The only thing I know is, don't rush, think about what is around you and what needs to be used. use cover, slag slag slag. Farm new gear when you must, if it gets too hard do CO-OP or walk away and try later. I think OP levels are not for everyone, just be sure you enjoy the game or don't do it.
No you dont. Grog is useful only for %age of healing and slag chance, neither of which change with level. Besides you cant get a new one. If you have the mission one, it levels with you. And there is no way to get a second once you turn the mission in in a given playthrough. How do you not know this?
