I think Stalker is pretty high on the difficulty scale for FPS games, even with the best weapons and armor you can still screw yourself over if you aren't cautious (e.g., a headshot with any low tier weapon can potentially kill even the most elite player).
I'd say Stalker is a game where thinking about tactics (where/how to engage an enemy) and consequently AI that can think tactically (uses cover, tries to flank) is where the crux of the difficulty lies, whereas Dark Souls is more about having good reflexes and split second decision making.
If you're looking for some extreme difficulty in Call of Pripiyat, then grab the Misery mod, it's amazing.
