V sync is useless, how do I fix this screen tearing
Why include v sync if it does not even work

Switch to the beta version (pre 1.0007 in steam game options), it has working vsync.
Disable 60hrz option. It causes the game to want to refresh at a different rate than what it actually is capable of. I was capable of over 300 FPS but only want 120. I set it in my Nvidia control panel to limit it to 120, but then noticed screen tearing. The fix was to disable 60Hertz option in game's settings. You can keep Vsync on after this. It's not broken, it's just a conflicting bug. Keep in mind that at the time of release for this game, getting over 60 FPS was basically unheard of in gaming, so this was not planned for. It's just a product of the game's age.
