Did you infact try the reload button? I recall it being changed to that one as not every mouse posses a mouse 3 button and thus setting it by default there would confuse a rather large market of player´s.
Personally I'm a bit pissed off about the sudden change from "Special Action button" (mousewheel) to Reload key (R).
Originally, the Vaccinator resistances were tied to the "Special Action" key, separate from everything else. In fact, Valve specifically added the "Special Action" keybind into the Options menu when the Vaccinator came out. It wasn't there before.
This allowed players to rebind that Special Action to some other key, if they wanted. But now, if I want to rebind the Vaccinator resistances cycling to something else, I'm rebinding the reload action for all weapons at once!
The "Special Action" still exist in the Options menu, but does it do anything anymore? (I don't think so. It's completely superfluous now.)
I am puzzled as to why they did it.
1) First of all, what gamer has a mouse without a mousewheel? (default Mouse 3 button)
2) Yes, having the mousewheel do two different things when you're playing Medic was a bit of a pain for me when the Vaccinator came out: clicking the mousewheel changed resistances while scrolling with it switches weapons, so in the beginning I often accidentally switched weapons. But I got the hang of it.
3) With a medigun you don't need a left mouse button all the time (unless you switch to another patient) so I could use the index finger to click the mousewheel while I used my left hand on my keyboard for dodging and moving. NOW after the change I sudenly have to used WASD and SPACE and R to toggle resistances at the same time. This is worse than before, if you ask me.
I remember, back when the Vacc came out with the Christmas 2012 Mecha Update, no-one had a clue how to access that resistance cycling! Until someone figured out that the new "Special Action" mouse 3 keybind that had appeared in the Options might be it.

I do believe you can actualy re asign the button at your lessure so in the case Mouse 3 was more favoritable to you you could simply re asign it to said button.
Weird. R works fine for me. You cannot switch between resistances while an übercharge is active, though. But as each übercharge-segment is only 2 seconds long, you can quickly switch between segments (unless you keep Mouse2 pressed the whole time).
That would keep 'r' bound to +reload as well as mouse3 being bound to +reload.
Also, besides if playing as a spy, why would someone not have cl_autoreload set to 1?
I use it as my action key.
