What do you think is in the syringes from the medic's syringe gun?
Naturally the other primary's use compleatly different or even fictional chemicals to cause their effects, but what is in his syringes that he shoots? poison? seditives? what ever they use in leathal injections? before you type some sort of disease, like aids, think about how long it would take for a disease to kill an enemy by killing their white blood cells.

for the stock they're just needles, but I'm pretty sure that the blutsauger and crossbow use different "needles" where I'm not completely sure what. but like tasty salamanders said, the blutsauger probably sucks your blood out, how it heals the medic? dont know. and I'm completely lost at the crossbow does more dmg from distance, hurts enemies, heals friends. maybe its some sort of virus the medic created? a sentient one maybe? I bet that would be something a crazy german doctor would do in his free time...

Stock just shoots empty, sharp needles.
Overdose probably shoots empty needles too, the speed boost comes from somewhere else.
Crossbow has actual medicine. Medic just quickly changes the healing needle for a poisonous needle if the target is an enemy. He's that fast.
Blutsauger has invisible, really long wires attached to them that connect them to the gun. Whenever a guy is hit, the needles absorb their blood and bring it back to the gun, where an off-screen tube transfuses it into medic.

I'd say this is logical.

Stock: Normal syringes like in actual hospitals with no medicine inside, just pure needles.

Overdose: Same as stock, the speed boost probably comes from within the gun and connects to the Medi Gun, not related to the needles.

Crossbow: I'd say medicine, but if it was normal medicine it would heal enemies as well. So I think it's based on the same energy that the Medi Gun uses and is put into a crossbow.

Blutsauger: Since you have a constant HP loss, and HP on hit, it's logical to say it has some sort of system based on blood. It requires blood in order to work, so if you aren't taking other peoples blood it takes your own in order to stay active. What it shoots is a mystery to me, maybe some sort of micro teleporter that transports blood via. the molecules in the air when it penatrates the enemies skin.

Crossbow: Medicine and a pathogen that deals more damage than the medicine can heal. All players in the same team have a vaccine, which means only the medicine works. Pathogen probably dies quickly.

Blutsauger: Quantum teleportation allows blood to flow into the medic's body through an unknown machine below his clothes.

Overdose: Same thing, but uses adrenaline instead of just blood.
