Wouldn't be too bad.
It actually wouldn't be a bad combat engi secondary. It would support the idea of survival over firepower just like the gunslinger.

The Pocket Pistol is intended for quick pickups of health in the middle of combat, which the Engineer will never be getting himself into anyways.

Gunslinger: Surviving over firepower (the crit combo is borderline useless and rarely used)
They both fit.
It would fit perfectly for him.
But if youre arguing that engi is only a defensive class than I guess the gunslinger, frontier justice, pistol are all pointless.
He doesn't have a deployable that gives him free health, so I feel like this would be a welcome change, as unlike the scout who is not a combat class and is therefore not taking a large risk by spraying all of his shots extremely quickly and having less of them (If you've never played 6s against similarly skilled players you don't get to contradict this since that's typically where the pistol will wind up mattering), the engineer is not designed around standing in one place next to a building that gives him lots of health and could therefore benefit greatly from a gun that generates lots of health, as well as his asinine 200 bullet pool which he gets because of course he does
Also worth mentioning: If you were to add this the scout could pick it up, meaning we'd have 200 reserve scouts with this thing, and despite what you might think this would actually be somewhat terrifying
