The Medic is a Geneva Convention violation
A red cross on a white background is for denoting non-combat medical personnel, and use protection applies to (but is not enforced for) video games.
The Medic is a combatitive unit and thus not allowed to use the symbol; not only is he not allowed to use it canonically since I assume the Geneva Convention is canon in TF2, he's not even allowed to use it as a video game character.
Pretty poor standards tbh Valve, violating the Geneva Convention with your character design

Fictional characters aren't real - someone at Valve sat down and deliberately designed a character to violate the Genva Convention.

Medic isn't a non-combat medical personnel. He's in the middle of combat ensuring the people around him keep shooting.

In the TF2 canon, Abraham Lincoln invented stairs, and before him, we got to the second floor by rocket jumping. I believe that it isn't hard to imagine the Geneva Convention not existing due to the fantastical technology developed during this time. Somebody probably market gardened Hitler back when he was a messenger boy in WW1, preventing the need for a Geneva Convention.

It’s only on a white background with some cosmetics based on real items
