And how to find them?
Just fight Dangerous Encounters (two enemies close together to start the fight, black aura on the ground) exclusively, and don't die / use items / etc. -- the goal is to get high grade. The higher grade you get in fights you raise the chance of getting a Dire Foe to appear. I've had no trouble finding them (not farming, just playing through the game naturally) because I almost always go for the Dangerous Encounters.
Not sure what the encounter rate of Intense is, but I'm sure it'll be intense.
Not hard to do if your spamming soul breaks like a monkey.
Believe me it adds up.
Aside from that, not getting KOs, not using items, using one Mystic Arte to kill an enemy, not being under a status effect at the end of the fight, all of those are fairly massive.
Difficulty influences things a bit, finishing very fast also gives a boost, bosses, red code and dire foes usually give significantly more than regular encounters (provided you don't die).
Soul break, status effects and stuns you and your team dish out make a tiny contribution each time, but after hundreds of fights it adds up to quite a bit too as said above.
Personally I'm not farming for it for now, but I'm definitely accumulating it steadily by fighting mostly everything I see since the start of the game.
Basically try not to die too much and to counter status effect as much as possible, and it's possible to get 2+ grade per fight.
Edit: I just went to check, I already have 3.3k Grade before I even have the geo board, that's how much I farmed the areas I guess.

Hahaha im in mount killaraus now and with the difficulty set to chaos + dark bottle. That guy tries to jack my party every 4-5 fights.
I wonder if im going to make the 13k grade before I clock the game.
How much grade do you earn when you do fight him?
