Question about sequence

After completing this sequence shown above the last screenshot shows the door that is supposed to be automatically unlocked.

In my game after completing all the steps shown above I arrive at the last screenshot. In my game this door remains locked. I will tell you that when the interface turned red (indicating the monster was after me) I ran back in the previous area and hid under the hospital bed. Afterwards, I came out and then proceeded to the door and the door was locked. I could no longer go back to the area where the password was found because it had changed back into a loo again. So, I simply wandered around all over with no indication from the game that I had failed or died.I believed there must be a solution not realising that I had somehow fallen into a curious glitch.I would have to end game and launch it again . I would then have to proceed forward through the red screen this time in order to reach what was now an unlocked door.
