Report a translation problem
Since RS2 models every pellet individually, there are no guarantees you will get a killshot, especially if someone is moving, prone or in cover. Sometimes I straight up drop people as fast as I can chamber shells, sometimes it takes me several shots to neutralize an enemy. Remember that even if your shots don't seem to be connecting, they may be causing minor or major wounds.
I use 00 buck for more open areas; it will kill much further than smaller pellets. 00 also penetrates quite a lot of cover.
#4 buck is my go-to for the closest and most congested areas; bunker systems, trenchlines, urban fighting, etc. A #4 shell has more pellets than a comparable 00 shell, but they're smaller and won't cause fatal wounds as far out as 00.

I aim right for center mass. Haven't had any trouble with the shotgun sights myself; it's not exactly a precise weapon. In a situation dealing with a bunker window, I might suppress with shotgun fire and follow it up with a grenade, just to be sure.
I mannaged to sneak up on the VC and mannaged to frag at least 13 players up close in An Lao Valley on Carlie. It was quite fun to use it as my broomstick.
I will create a proper fps guide for this game soon to help people "git gud" like I see some people write.
I use all three, kinda depending on where I intend to fight. Just like I use all three Ak's, and pretty much everything except the stock french variation of the NVA submachine gun.
Also just practice how to work around that situation if you get caught in it. Flanking, reatreting and pulling them in an ambush. I f you have a mic and a fried, cover fire actually compensates, as it forces an enemy to either evade, or return fire, both of which takes his focus off you. You just need to be quick, as some can recove rather fast.
Or it could just be bad luck with that particular engagement. Or it could be the hitreg bug that appears from time to time, but that's also easily confused with ping/lag issues, pellets merely wounding an enemy and a bunch of other possible situations.
