You also dont need to lose many points if you do not have alot of unity. With all the Islands you have you can lose so points there without even knowing. France has the same problem and can flip without taking the mainland if you control all their Islands.
Also if this is a multiplayer game you could have lost by paratrooper sniping.
It can't be that I own the Whole of Italy and all of a Sudden Italy is Capitulated to Americans that Where not even close to borders.
I had some landings I didn't notice for a while being busy beating Soviets, so that might be it.
