Vichy is a decision after conquering France, Slovakia is part of occupying Czechoslovakia and Germany has a few choices on what to do with them.
For the world tension thingy its 50% for democracies, 100% non-aligned, fascist and communist states seem random but will usually join a faction if attacked.

1. Peace conference. Select a nation and use satellite (for nations they have puppeted or can release) or puppet (for them)
2. By event or focus. Some nations have events and/or focuses that will let you puppet someone.
3. Releasing as puppet. Any nation you have previously annexed can be released as a puppet (might need TfV for this)
They may not be called puppets, but once an occupied nation gets a high compliance etc you get a message saying they are ready for next step and will you give your blessing for them to form a new government that is collaborative. (If you chose "they are not ready" you can always go into politcal decisions later and chose to do it. I think it is free.)
If you do it, you get % of factories for your use and you can do sweet trade deals to get resources - like 80 iron for 1 civ etc. but they are basically a puppet and have their own military etc.
I view it the same as a puppet at this point, not sure the entire difference.
Anyway this starts their journey toward independence and you can control that through the manage subjects button.
As they rise through the next 2 levels they get more independent. You can force them back down to annex by lend leasing them a bunch of stuff until they hit a certain threshhold, or you can let them send you all their resources and they will gain independence over time. Keep an eye on that bar if you don't want them to get independent where you lose the factories etc.
