Report a translation problem
"Failed to create a graphics device.
This might be because you have more than one screen/graphics card.
You could try to write adapter=1 in the settings.txt file."
But I do not have a solution...
Either way please report back how you fixed it for the benefit of other users.
If you don't see your settings file you can always create one. Open notepad and create a new file called "settings" and paste the text from the following link (and then add the extra line):

The new file you've made should then be saved in Documents\Paradox Interactive\Hearts of Iron IV
Recently I hooked up two monitors and I found a solution:
Go to the settings.txt file and make sure you are standard windows mode, borderless does not work. I have two 1920x1080 monitors on this machine, so I set my resolution for 3840x1040 (The less vertical resolution is account for the title bar). This will give you a windowed screen but it will fit across both monitors. This is not a perfect solution but it does work.
The settings.txt I use:
