If that indeed is the only mod used then check the discussions of that mod if others are experiencing similar issues or ask the mod devs there.

Click the "DLC" tab before clicking the HOI4 game "Play" button. Then expand "Content" by clicking the green arrow just to the left of "Content". Then click on "Historical German Portraits" to enable the portraits (it will show a green check mark in the same line as "Historical German Portraits" and a solid green box in the "Content" line when portraits are enabled).

Also, I have seen the blank portrait as well especially Catholic Mexico at the point when the Second Cristiada War broke out
