If you go the monarchy route and want to federalise then you need to take over the old dominions WITHOUT using insurection.
You only gain cores on the dominions, using the federation focus, if the dominion in question is the ORIGINAL dominion. By going with insurections you spawn "new" ones even though they take over the tag later on.
On top of that, you need the various nations to agree to the federation for you to gain their cores. Best done before getting claims on all their territory as that wrecks your relationship.
Btw, don't use the focus until after you have taken over the US, there is a decision to unite canada and the US is put under the canadian tag. If done you will get the US as a core as well with the focus.
